If you do not have a GotSport account, please ask your club/org registrar to add an Email/User ID for the player and/or parent/guardian (if the player is under 18 years old)

• • As a Parent – Adding a Photo to a Player Profile
  • Add proof of birth documentation (for one-time verification by US Club Soccer)
  • Complete the US Club Soccer Form #R002 (Player Info, Medical Treatment Authorization, Liability Waiver/Release and Consent Form)
